Pricing Conditions

This Terms & Conditions policy governs all pricing, financing, insurance, and promotional offers presented by Diamond Braces. By engaging with Diamond Braces services or offers, patients agree to the following terms.

1. Final Treatment Plan Determination

All final treatment plans are based on an in-person clinical examination and review of diagnostic records conducted by a licensed orthodontic doctor. Virtual consultations and online appointments are for informational purposes only and do not constitute a finalized treatment plan.

Prior to finalization, the following must occur:

  • In-person clinical exam

  • Review of x-rays, 3D scans, and/or other diagnostic records

  • Confirmation of medical history and treatment candidacy

No pricing, financing, or treatment guarantees can be extended until the above process is completed in full.

2. Pricing and Treatment Term Lengths

2.1 Standard Treatment Pricing

Comprehensive orthodontic treatment pricing is structured for treatment terms of up to 24 months unless otherwise specified. Costs are calculated based on:

  • Case complexity

  • Patient medical status

  • Age

  • Compliance with treatment

  • Tooth movement variations

Failure to comply with treatment instructions or required appointments, as well as unforeseen changes in case complexity, may necessitate an extension of treatment beyond the original term. Additional fees may apply if such extensions occur.

2.2 $89/month with insurance

  • Reflects an estimated out-of-pocket cost for patients with active orthodontic insurance coverage. Actual monthly payments vary depending on plan benefits, insurance approval, and treatment specifics.

2.3 Additional Pricing Disclosures

  • Pricing subject to change. Promotional offers cannot be combined with other discounts. Additional fees may apply for expedited shipping or replacement aligners if needed outside of the original treatment plan.

2.4 Invisalign & SMILIFY Treatment Pricing

  • Comprehensive Invisalign Treatment: Includes full prescribed set of upper and lower aligners for a term of up to 24 months or until the originally prescribed treatment is completed.

  • SMILIFY Expedited Invisalign Treatment: Includes prescribed set of aligners for a term of up to 12 months or until the originally prescribed treatment is completed.

SMILIFY Expedited Invisalign Treatment is only available to qualified candidates following in-person clinical evaluation and review of x-rays. Eligibility is determined solely by Diamond Braces doctors. Not all patients will qualify.

Savings associated with SMILIFY cannot be combined with other promotions unless explicitly stated otherwise.

2.5 Additional Pricing Disclosures

  • Pricing subject to change. Promotional offers cannot be combined with other discounts. Additional fees may apply for expedited shipping or replacement aligners if needed outside of the original treatment plan.

3. Lifetime Smile Maintenance Guarantee

The Lifetime Smile Maintenance Guarantee is available only to patients who:

  • Have completed treatment through Diamond Braces

  • Are actively placed in retention by the Practice

4. Promotional Offers & Financing Terms

4.1 “$0 Down,” “0% Financing,” and Flexible Monthly Payment Plans

"$0 Down with financing" available to qualified applicants only. Credit approval required through third-party financing partners. Not all applicants will qualify. Monthly payments, interest, and fees may apply depending on creditworthiness and selected terms. These financing offers are available only to qualified applicants. Eligibility is determined based on:

  • Financial review conducted by Diamond Braces’ financing partners

  • Creditworthiness

  • Applicable insurance coverage

Approval for financing is not guaranteed for all applicants. All offers subject to in-person clinical evaluation and finalized treatment plan by a licensed orthodontist. Virtual consultations do not constitute a final quote or plan.  Additional fees may apply for extended treatment, replacement retainers, or missed appointments. Restrictions may apply. Cannot be combined with other promotions unless explicitly stated

4.2 CareCredit Disclosures

CareCredit financing is subject to credit approval. Minimum monthly payments are required. For full details, visit carecredit.com. Diamond Braces is not affiliated with CareCredit. Promotional financing options may be available for qualifying purchases.

4.3 Additional Terms for Promotions

Unless otherwise stated, promotional savings, including those associated with SMILIFY or other limited-time offers, cannot be combined with other offers or discounts. Restrictions may apply to specific promotions.

5. Insurance & Coverage Disclosures

  • Insurance coverage for orthodontic treatment varies by provider and plan. Diamond Braces does not guarantee insurance reimbursement. Please contact your insurance provider directly to verify coverage details and reimbursement process.

  • Final pricing may vary based on applicable insurance coverage, benefits, and patient eligibility.

  • Diamond Braces cannot guarantee insurance coverage or reimbursement. Patients are responsible for verifying coverage through their insurance providers.

  • All out-of-pocket costs, deductibles, and co-pays are the responsibility of the patient.

6. FSA / HSA Eligibility

Most FSA (Flexible Spending Account) and HSA (Health Savings Account) administrators consider aligners as eligible expenses. Eligibility and requirements may vary by plan. Patients should consult their HSA or FSA administrator before purchase. This information is not intended as tax advice.

7. Treatment Term Disclaimer

Treatment term lengths and associated pricing are calculated based on each patient’s unique medical condition and may vary due to factors such as:

  • Age

  • Case severity

  • Medical history

  • Patient needs

  • Chief complaint

  • Treatment compliance and progress

Diamond Braces reserves the right to adjust treatment timelines or associated costs based on these factors.

8. Policy Updates

Diamond Braces reserves the right to modify this Terms & Conditions policy at any time without prior notice. The most current version will be posted on www.diamondbraces.com.
